IMPORTANT! Nurse Aide I Refresher. You MUST have the following on the day of your registration appointment for Nurse Aide Refresher to register: Payment of $145.00. Verification of previous NA I Registry within the past 5 years. High school diploma or High School Equivalency Diploma or take the Reading Placement Test Two forms of matching ID (photo driver's license and signed social security card with no lamination). Current American Heart Association BLS card. You must register to take the class at (9190 209-2522 if you are not certified (must be completed before the first day of class).
